If you read the report carefully, you will see that the BA SOPs call for the captain to operate the fire handles and the FO to operate the fuel cut offs simultaneously, so as to speed up the process. Thus no criticism is to be attached to the pilots . This is the reason for the modification which is gradually being introduced, splicing the two circuits together

Hi Chris,

I see what you're saying and the reason for Boeings modification is understood. The investigators found the run/cutoff valves still open and therefore, the fire handles had been pulled prior to the fuel control switches so while attributing no blame.. the sequence doesn't appear to have been 'simultaneous'. Simultaneous seems a pretty ludicrous expectation anyway given the reactions of two people are not going to be exactly the same.
