As I mentioned above......the take off was made with the controls locked

To unlock them ment pulling the power off but in their shortness of thinking time that was not done. The gust lock lever was found embedded in the pilots hand.

Here's the incident.the cause I read about elsewhere

It reminds me of a reading about a DC 6 crew in the States that saw a brand new Canadair North Star

They'd not seen one before so approached the crew and said that it looks like a DC 4 with Allisons

They soon put them right by explaining they were Rolls Royce Merlins

"Ah yeah Packard Merlins" the American crew responded

"Nope Rolls Royce Merlins..... Rolls Royce, Derby, England"

"Hell they making them over there now!!?"

Horrible, how tragic... A crucial pre-flight check overlooked - it happens occasionally. :-(

A video like that probably shouldn't be shown, but, there you go.

I think it is one that needs to be shown if only because it doesn't show anything patently graphic (like bodies which are evident in the MiG-29 airshow crash video available from many sites). It is a sad and shocking reminder of what happens when you don't use the checklists. Sometimes that's what it takes to save another life. We lost a Convair for the same reason - crew didn't follow the checklist and got busy doing other things and the co-pilot died because of it. The pictures of the aftermath really woke up quite a few people within our own company, not to mention many other airline crews. When you have an accident, it's important to remind people of what happens when you get complacent.
