It's worth reading that report in conjunction with the video clip I linked above. I believe most (if not all) of the incidents mentioned in the report are shown in the video.
The synopsis by the USAF Major of what led up to this tragedy is a chilling document : without in any way being flippant, it demonstrates absolutely the truth of the adage about old pilots and bold pilots, and reading it made me want to weep, because it was all so unnecessary

Canberra's only had one flaps setting, didn't use flaps on t/o, only landing so.....I fink it may have been a messed up go-around...followed by a stall/wing drop as Tonks says.
