britishtourer, do you read the AAIB accident reports? They publish bulletins every month, which are a wealth of information to those of us who are interested in such things. They will really help to build your awareness and understanding of the way things go wrong in aviation.

You will probably find 9 instances of human error for every equipment failure! It's interesting reading! Have a look:
